New Patient Registration

your gp websiteIf you live in the practice area and you would like to become a patient of the practice, you will need to visit the reception in person to collect a form called a FP1 which you should complete and return to the reception.

Once you have returned the form it will be given to one of the doctors who will sign it agreeing to accept you as a patient. If the doctor refuses to sign the form (The most likely reason for this would be that you do not live in the practice area.) then we will not be able to accept you as a patient of the practice but we will help you with any urgent medical attention you require.

Once you have registered with the practice the local Primary Care Trust will be informed and your medical records will be requested from your previous GP. Please note that it may take several weeks for us to receive your old medical records.You will also receive a medical card from the Primary Care Trust showing that you are registered with one of the doctors in the practice.

Please note that you may see any doctor within the practice not just the doctor you are officially registered with.
